		Liability Insurance - $15.00
		Uniforms (uniforms must be worn when students are in clinical training).	$23.00
		White Shoe - any type of white leather or canvas shoe will be acceptable	$50.00

		B.  Entry Requirements:
		A current TB skin test or chest X-ray is required.  (Skin test may be obtained at your local  Health Department.  Call for an appointment.)	$9.21
		Hepatitis B Vaccine is strongly recommended.  (Bedford County Health Department charges $45.70 per shot and it is a series of three shots.)	$137.10

		C.  Class Meeting times:
		Tuesday, Wednesday, Thursday
		4:30 p.m. to 7:30 p.m.
		D.  Certification Test
		Upon successful completion of the program, your instructor will schedule you for the certification exam.	$85.00
